How to verify your coverage

Close-up of insurance paperwork and documents on a desk

1) Share your insurance details

Send a photo of the front and back of your insurance card, plus your date of birth.

2) We confirm network & benefits

We’ll check eligibility, primary care assignment (if applicable), and expected copays or deductibles.

3) Schedule with confidence

Once verified, we’ll book the best appointment type—annual physical, sick visit, vaccines, or follow-up care.
