At Onestop Medical Center, our Sliding Scale Fee program ensures that patients with lower income levels pay lower medical fees, making healthcare more accessible and stress-free. After you submit a completed application along with the required supporting documents, our team carefully reviews your information to determine your eligible discounted fee. This process helps us provide fair, transparent, and affordable healthcare services for every patient.
Sliding Scale Fee : What's Covered ?
Our Sliding Scale Fee applies to:
- Primary Care Visits — Sick visits, routine checkups, and follow-up appointments
- Preventive Care & Screenings — Annual physicals, vaccinations, blood pressure checks, cancer screenings, and more
- Chronic Disease Management — Ongoing care for conditions like diabetes, hypertension, asthma, and heart disease
- Behavioral Health Services — Individual therapy, counseling, and support for mental health and emotional well-being provided by our licensed behavioral therapist

Your total gross annual household income (all household members combined)
The number of people in your household
No insurance? No problem—you are still welcome here.
How to Apply
Download and complete the Sliding Fee Discount Program Application
List all household members—including name, date of birth, relationship to you, and whether each person has health insurance
- Recent tax return, W-2, or 1099
- Recent paycheck stubs or employment letter
- Self-employment records or Profit & Loss statement (3 months)
- Social Security, pension, or retirement statements
- Public assistance, unemployment, or disability documentation
- Child support, alimony, or workers’ compensation records
- Any other source of household income

Can I use the sliding scale for behavioral health visits? Yes. Behavioral health services with our licensed behavioral therapist are included in the program under the same sliding scale fee.
