We offer two simple ways to make your gift:

Pay by Check Make your check payable to "Onestop Medical Center" and mail it along with your completed donation form to:

Pay by Credit Card We accept Visa, MasterCard, American Express, and Discover. Simply complete the donation form with your credit card details and mail it to the address above.
Tribute Gifts
Make your donation in honor of or in memory of someone special. Simply indicate your tribute preference on the donation form, and we will acknowledge your gift accordingly.
You may also choose to remain an anonymous donor — just check the appropriate box on the form.
Designate Your Gift
Unless otherwise specified, all contributions are allocated where the need is greatest. If you wish to support a specific program or service, please indicate your preference on the donations form.
